What about the ums.deny file.
It was broken quite a few years back but if I remember correctly, about 2 years ago work started on trying to fix it.

I thought the deny file was just to deny access to IPs but I don't think it works anymore, not sure as I have never used it.

Just reading this from the knowledge base it does seem that is part of its function but its not exactly user friendly to set up.
This is one of the things that should be worked on for V14 then it can become part of the web GUI.
Logs are important for us to help, Please follow This Link before asking for support. Just a forum cleaner, Will help if I can but no expert.

If UMS.deny was working I think it would be perfect for this type of thing.
It looks like it works by using "regex" (regular expression) which can be quite powerful.

To exclude a file(s) show you could:
Add individual files.
Add any file that had certain text or part of text in the name in specific folders or all folders.
Add files with a specific filename extension.

I will test it and see if it has been fixed and working again.

Edit: I tested it and it is not working.

A folder with random files. Sometimes sub-folders.
example file extensions are .nfo .jpg .png
e.g. backdrop.jpg and folder.jpg and logo.png
also sub-folder could be named metadata and have another file like .jpg in it.
