I suspect this is a render thing. Are you using a PS3? If so I've seen it from time to time pick up "half" of the stuff I know it's there and then after "clicking" around a while it suddenly finds all the stuff. My BRAVIA (as well as XBMC) finds new stuff directly. Can you try running for example ...

Are you using the media library cache? If so, try disabling it
I don't think so. The default is false and I haven't specified anything on that line of the .conf file.
Seems to be consistent with UMS and PMS as to what files aren't refreshed. I wish I could pin point the issue, but my gut feeling ...

You could try telling UMS not to transcode that file type. I used this for my renderers for .mp4 & m4v extensions.
Also are you running on Windows, linux or Mac?
